Review doses in renal impairment

TAZOCIN 4.5g iv tds (monotherapy)
If definite history of rash allergy but no anaphylactic penicillin allergy (see explanatory notes), use:
MEROPENEM 500mg iv tds
Treatment duration: typically up to 5 days

TIGECYCLINE 100mg iv stat as loading. Then 50mg iv bd (25mg bd in severe hepatic disease) plus
GENTAMICIN iv (as per local protocol)
