Surgical Prophylaxis - Vascular Surgery
Aortic Aneurysm, Femoral-Popliteal Bypass Graft, Amputation

CO-AMOXICLAV 1.2g IV single dose at induction then IV tds for 24 hours
OR
FLUCLOXACILLIN 1g IV single dose at induction then IV qds for 24 hours
plus
GENTAMICIN 1mg/kg* (max. 80mg) IV single dose at induction then IV tds for 24 hours
If current / previous MRSA positive:
Add
TEICOPLANIN 600mg IV at induction then 400mg at 12 and 24 hours after surgery

OR
If penicillin allergy (explanatory notes), then use:
TEICOPLANIN 600mg IV at induction then 400mg at 12 and 24 hours after surgery
Plus
GENTAMICIN 1mg/kg* (maximum of 80mg) IV single dose at induction then IV tds for 24 hours

*Add metronidazole 500mg IV tds to provide cover for anaerobic infections, particularly in patients with diabetes, gangrene or undergoing amputation
