Addenbrookes Cognitive Examination: This 100 point Addenbrooke's Cognitive Examination provides one score of "cognitive function". It includes within it the "mini-mental" 30 point score and takes about 15 minutes to carry out. Please note that it does not replace full cognitive assessment by neuropsychology. Local dementia services
Assessment of suspected dementia can occur either through the Neurology and Health Psychology service or via the 2gether memory assessment service. There are no rules over which service to use for which patient though neurology would see itself as primarily offering a diagnostic service.
Patients can also be referred to the countywide memory assessment service run by the 2gether trust, by single point access: Their process is referral, then RMN assessment, then Consultant assessment. Referrals should be made by letter giving:
• Why the person is referring
• Relevant medical history
• Medication (if any)
• Results of screening bloods FBC,LFT,U&E, B12 ,Folate and TFT’s
• Physical examination neurological exam (if indicated)
• Whether the person is aware of the referral and who is the point of contact to set up an initial appointment
• Where the person would like to be seen
In addition cognitive examination information e.g. 6CIT, Addenbrooke's Score, Mini Mental State Examination is helpful.
